Medical and Religious Exemption Request Form
The message below was sent to the Dartmouth community on April 11, 2023. Dartmouth is no longer requiring documentation or exemption from the COVID 19 vaccines; therefore, this form is no longer accepting requests.

Dear students, faculty, and staff,

With the U.S. public health emergency designation poised to end shortly, we want to announce a change to Dartmouth’s COVID-19 vaccine policy and, at the same time, to reinforce that COVID-related resources will continue to be available at Dartmouth.

Beginning today, April 11, Dartmouth no longer requires documentation that students, faculty, and staff are vaccinated against COVID-19 or have an approved exemption. Dartmouth continues to strongly recommend that all students, faculty, staff, and visitors stay up to date with their COVID-19 vaccines according to guidance provided by the U.S. Centers for Disease Control and Prevention.

Dartmouth will work with health-care providers, including the Dartmouth College Health Service, to continue to keep COVID-19 vaccines available on campus and to share information on finding vaccines in the community. Free face masks and COVID-19 antigen tests will also continue to be available for Dartmouth students, faculty, and staff at pick-up locations around campus. We encourage you to have at least two tests and a few masks on hand in case you have symptoms, test positive, or are exposed to someone with COVID-19 and need to wear a mask and test.

If you do test positive for COVID-19, isolate, wear a mask around others, and talk to your health-care provider, as you may be eligible for treatment. Students who test positive should contact the Dartmouth College Health Service by emailing dicks.house.nurse@dartmouth.edu or calling 603-646-9440 and notify their close contacts. Faculty and staff should call Axiom Medical at 833-408-1338 to report their positive test. Axiom will notify close contacts who are Dartmouth faculty or staff. Faculty and staff must notify all other close contacts, including Dartmouth students.

If you have questions about your health, including persistent symptoms that may be a result of long COVID, contact your health-care provider. Students should contact the Dartmouth College Health Service. Dartmouth resources for coping with COVID-19 and long COVID are also available.
